Tuesday  –  8 am – Noon

Wednesday & Thursday  –  8 am – 4 pm (closed Noon-1pm)

Services Offered:

  • Immunizations – Infant, early childhood and adolescent up to age 19 yrs.

  • Vision & Hearing Screenings

  • Lead Poisoning & Iron Screenings

  • Referrals as needed

  • Assistance with accessing Medicaid

We accept all forms of Medicaid, including CareSource, Molina, United Health Care, Buckeye Health Plan, Anthem, Humana Healthy Horizons and AmeriHealth Caritas Ohio.

For those who do not have insurance we offer services on a sliding fee scale that considers family size and income. The sliding fee scale allows us to provide services at a reduced rate, making them affordable for all patients to receive the necessary immunizations.

No one will be denied due to inability to pay.
