Please send completed form to brookealverson@coshoctoncounty.net so that a physical and drug screen may be scheduled with the Coshocton Regional Medical Center’s OccMed office located at 1523 Walnut Street.<\/p>\n

Please have all new employees complete the top portion of the background check and send it to brookealveson@coshoctoncounty.net, this background check will take place at the Coshocton County Sheriff’s office.<\/p>\n

Once I receive all completed forms I will schedule all testing and call the new employee with the date and time of their physical, drug screen and background check. Please make sure to include a phone number in your email to me.<\/p>\n
