Program Description
The child health center offers well child physicals and immunizations for children birth to 12 years old. A registered pediatric nurse and a certified nurse practitioner, as directed and reviewed by a medical doctor, provide services. These services include the following:

Review of medical & social history

Physical exam

Development screenings
-
Review of nutrition & growth
-
Screening for blood lead level
-
Speech, vision &
hearing screenings

Immunizations
-
Assistance with social services
Medicaid is accepted, or fees are based on income and family size. No one will be denied services because they are unable to pay. Please bring proof of income, medical card, and the child’s immunization record
